Passport Services

Changing your name in your passport

Your passport may be amended to show your married name or a legal change of name. Processing time is only a few days.

Here's what you'll need:

  • Application Form
    Please complete an application form (DS-5504). Note that the online version of this form includes instructions on the back side written for use in the U.S. which do not apply to your application in Fiji, Kiribati, Nauru, Tonga or Tuvalu.

    You will need to use the latest version of free Acrobat software to view and print this form. You can download the software here.

    Note that the form you print must be clear and sharp, with black print on white paper. The paper must be 8 1/2 inches by 11 inches. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.

  • Documentation
    You'll need to show us legal documentation supporting any requested change.For example, your original marriage certificate showing your new spouse's name or an original court order showing a legal name change.  We cannot accept copies, and we reserve the right to request additional documentation.

Please send us your passport and materials, along with a self-addressed, Y270 stamped envelope.

Liliu ho hingoá ‘i ho’o paasipooti

‘E lava ke liliu ho hingoa ‘oku ‘asi ‘i ho’o paasipootí ke ‘asi ai ho hingoa malí pe ‘i ha hili ha liliu fakalao ho hingoá.  Koe ngāue ki hení ‘oku lau ‘aho pē.

Ko e ngaahi me’a eni ‘e fiema’ú:

  • Foomu kole

Kataki ‘o fakafonu ‘a e foomu kole liliu hingoá (DS-5504).  Fakatokanga’i ange koe foomu ‘oku ma’u atu mei he ‘initanetí ‘oku ‘asi he tu’a foomú e fakahinohino ki hono ngāue’aki e foomú ni ‘i ‘Amelika, ‘a ia ‘oku ‘ikai fiema’u ke ngāue’aki e fakahinohino ni ‘i Fisi, Kilipati, Naulu, Tonga pe Tuvalu.

‘E fiema’u ke ke ngāue’aki e polokalama ‘oku ma’u ta’e totongi atu pe heni koe Acrobat kae lava ke ke lau mo paaki ‘a e foomú ni.

Fakatokanga’i ange koe foomú kuo pau ke lanu ‘uli’uli e ngaahi mata’itohí hono paakí ‘i ha pepa lanu hinenhina.  Kuo pau ke ‘inisi e 8 moe konga fālahi pea ‘inisi ‘e 11 lōloa ‘a e pepa ‘e ngaue’akí.  Ka ngāue’aki mai ha fa’ahinga pepa kehe, pe pepa ‘oku ngingila, ‘e ‘ikai tali ‘a e foomu kole ko ení.

’E fiema’u ke ke fakahā mai kia kimautolu ‘a e ngaahi pepa ke fakamo’oni’i’aki ‘a kole lilliu hingoá.  Hangē ko ha’o ‘omai ‘ae tatau mo’oni ho’o tohi malí ‘oku ‘asi ai ‘a e hingoa ho malí pe ko ha tatau mo’oni ‘o ha tohi tu’utu’uni mei he Fakamaau’angá ‘oku ‘asi ai ‘a e liliu fakalao ho hingoá.  ‘Oku ‘ikai ke mau tali ‘a e hiki tataú (copy) pea ‘e lava pē ke mau toe kole atu ke ‘omai mo ha ngaahi pepa kehe ‘o tānaki atu ki he ngaahi me’a ‘oku hā atu ‘i ‘olungá.

Lī mai he meili

Kātaki ‘o ‘omai ho’o paasipootí fakataha mo e ngaahi me’a ‘oku fiema’ú mo ha sila kuo ‘osi fakatu’asila kiate koe, ke fakafoki atu ai ho’o paasipootí.
